SportsOhio General Registration Form Guidelines:
Review the following information before completing your
registration and payment. The general registration form is for programs at
Soccer First, The Golf Center @ SportsOhio, Field Sports and PowerPlay. You may
register one (1) participant for up to four (4) programs on the same general
registration form. Each participant requires a separate form. Fill out all
information completely and legibly on each registration form. The SportOhio
Waiver and Release of Liability MUST be signed by the participant or parent/guardian
in order to participate. If paying by check, each sports venue/facility requires a seperate check. For example if you are registering for two (2) programs: KinderKickers at Soccer First and Flag Football at Field Sports, then two (2) checks should be written. The check for KinderKickers should be made payable to Soccer First and the check for Flag Football should be made payable to Field Sports. Registrations may be submitted in person or by mail. Registration Information:If paying by
